Insurance act payment notice | Public Guardian and Trustee of British Columbia

When insurance proceeds for a minor are sent directly to a named private trustee, the insurer has a duty pursuant to s.88 of the Insurance Act to notify the Public Guardian and Trustee (PGT) of this payment.

Each file must be under 25 MB, and the form has a 150 MB limit for all attachments.
